All minors are required to be accompanied by their parent (or legal guardian) for the piercing or tooth gem application. Parents will need to bring their Photo ID, as well as the minor‘s birth certificate to have any piercing or tooth gem performed on the minor. IF the minor has a legal state photo ID (such as a permit or drivers license)
All minors are required to be accompanied by their parent (or legal guardian) for the piercing or tooth gem application. Parents will need to bring their Photo ID, as well as the minor‘s birth certificate to have any piercing or tooth gem performed on the minor. IF the minor has a legal state photo ID (such as a permit or drivers license) and the last names, and addresses match for the minor and the parent signing consent, those two forms of ID will be accepted in lieu of a birth certificate.

We will also require the birth certificate of the minor, or, if the minor has a legal state issued photo ID, (such as a permit or drivers license) that the last name and address match the guardianship paperwork, that can be used in lieu of a birth certificate.
